Parent/guardian agrees that his/her signature below indicates permission for the child to attend virtual culinary instruction.
Participant agrees that their signature below indicates they understand that they assume full responsibility for personal injury and/or damages to personal property occurring during or as a result of participation in a virtual workshop.
I understand that I am fully responsible for supervising my child/children during the workshops.
The Camp reserves the right to dismiss any child whose conduct or influence is, in the opinion of the Camp, unsatisfactory or inimical of the best interest of the program.
Parents/guardians hereby consent to the reproduction and use of camper’s photograph, image or other reproduction for the purpose of promoting or advertising BHEC as well as the general promotion of summer camps through American Camp Association or UJA Federation.
I understand that culinary activities come with uncertainties beyond what my child may be used to dealing with. Such risks include using knives, ovens, pots, pans and other equipment. I am aware of these risks, and agree to supervise my child to ensure their safety and I am assuming these risks on behalf of my child.
It is agreed that any dispute or cause of action arising between the parties, whether out of this agreement or otherwise, can only be brought in the Columbia County Supreme Court located in Columbia County, New York and shall be construed in accordance with the laws of New York.
Parent/guardian acknowledges and agrees that his/her signature below represents that he/she has authority to enter into this contract and assumes responsibility hereunder.
